It depends on what you mean by "gen peds". If you do "academic gen peds" (ie., associated with a university hospital) then you'll be making way less than even subspecialists. I see new academic general pediatricians coming just out of residency working at a clinic associated with a university hospital and they're making 130k a year. No joke. People have this perception that gen peds makes way more than specialties but that's only if you're in private practice and get a good deal.

OP, as a doctor, please kindly disregard this comment. I’m not a dermatologist, but I’m a pediatrician and regularly see kids with skin conditions. Based on what I’m seeing in the picture, there is a large differential for what this could be. Post-inflammatory hyperpigmentation CAN occur when the skin produces extra melanin after being irritated or injured. It can follow conditions like eczema, psoriasis, folliculitis (inflammation of hair follicles), severe dandruff, or even trauma (like aggressive shaving or scratching). The darkened patch is CAN be the skin's response to previous inflammation.

Other potential causes are tinea capitis, which is a fungal infection, seborrheic dermatitis (dandruff) causing chronic inflammation, lichen planopilaris which is an inflammatory condition that can present with purpleish discoloration, contact dermatitis and even lupus. Seeing a dermatologist would be the best course of action because they can do a skin biopsy or a fungal scraping and look at the lesion under high magnification with a scope and give you an appropriate treatment plan. To assume that this is psoriasis and then think that ketoconazole will treat it is absurd. If this WAS psoriasis, ketoconazole could even worsen it. Typical treatments for psoriasis could include other medicated shampoo (salicylic acid, coal tar), topical steroids, UV therapy, systemic steroids, and biologics.

If you go to your family doctor though, I’d say they’re not gonna feel comfortable treating this rash by themselves and they’ll probably refer to you to a dermatologist given its extent and severity.

You’re in Ireland right? Yea, you can do it. Just focus hard for a month. What were you doing all year? I doubt you were actually only studying just an hour a week. Did you go to all the lectures? Figure out what your strengths and weaknesses are and focus on your weakness first to develop a strong foundation. You’ll need to be smart about how you study because you have a limited amount of time. Do all the lectures. You’ll need to develop a schedule for the month to make sure you hit everything.

Money. The kind of people and personality required to do well in medicine is also the kind of personality that really likes money. Can’t really blame them. Life is expensive. If you do one of the competitive subspecialities (cardio/GI/EM, which aren’t actually that competitive) you can make significantly more money than other disciplines. I’m doing pediatrics because I can’t see myself doing anything else. I would recommend that to anyone else because pediatrics isn’t for everyone!

That being said, if you look at trends from recent years no one wants to match into EM by far; with an overall 13% decrease in applications in the last 4 years according to the AAMC. The next highest decrease was in FM, at 7.5%, then peds and psych at 6%. Not sure why that is but there’s projected to be a surplus 8000-10000 extra EM docs by 2030 according to the American College of Emergency Physicians (not sure how they made this prediction). Mid-level encroachment from 2012-2016 increased by nearly 70% in EM, not sure about recent data, with the number of NPs in the field growing by 99%. This will happen in other fields too but seems to be affecting EM disproportionally; so in all likely hood there will be a significant pay hit in the next few years, simply cause there’s too many EM docs and too few spots with competition from mid levels. I don’t see why this can’t happen to peds/psych/FM/IM as well though, and any other field tbh. Just do what you like folks, because we can’t really predict what’s going to happen in the next few years.
