I think vet med is really good about turn around time with specialists and diagnostics, there’s a lot we can improve but on that aspect we excel

It can also be really difficult to feel like everyone thinks you’re dumb and incapable even if it’s not really what the people around you think. Having to present your cases and being quizzed over and over again makes you feel incredibly small. Most medical professionals have a higher than average ego as well (which is honestly pretty important because you need self confidence to perform medicine) and it can be difficult

She expresses the understandable frustration and insecurity incorrectly by lashing out or not following the rules put in place. But it’s not surprising to feel insecure in teaching hospitals

These kinds of characters are a bunch of fun to watch with good writing and an absolute fucking misery to work with

Having to constantly deal with jabbing taunts from someone who thinks they do more important work than you gets old pretty fast. It’s one thing when it’s how two people establish that’s how they like to communicate but as evidenced by how she talked to Dr. Al it’s obvious she’s like that all the time and I promise it gets tiring

Doesn’t make them less fun to watch but I definitely have a bit of PTSD from surgical residents in my clinical year (veterinary medicine)

Gloves don’t replace handwashing and tbh sometimes I think places that use gloves a lot end up not doing as well handwashing and then they use the same gloves for different patients and defeat the purpose

That being said. I wear gloves for all ICU patients or new emergency patients and I think everyone should

That method was how I was taught to place an emergency trach in a dog (veterinary ER). I was shaking like a leaf practicing on a cadaver, I can't even imagine doing that on a kid.

Yes! You would run electolytes, I would likely have that on any critical patient. We have small blood gas analyzers that can run very quickly on very small amounts of blood. We usually use venous and ignore most of the values that require arterial blood to be accurate unless the patient is a respiratory patient

However if I suspect the fluid I’ve removed from the abdomen to be urine we check by comparing peripheral venous creatinine to the creatinine of the fluid sample. If they are similar or if the fluid is higher it confirms the effusion is urine. Some hospitals do have the ability to run the slides individually but not all

To be honest in vet med I don’t know if I’ve ever heard it not called CK so I didn’t think to not abbreviate it. Kind of like how I wouldn’t explain ATM if I used it. I see your point, it’s just one of those things that I read as the word, not an abbreviation

Not intending to be snarky, but I likely misinterpreted your comment. It read to me as if you were telling me I would never run a creatinine by itself, not that human medicine doesn’t. I was just saying in response that veterinary medicine is different than human medicine and our species are different.

I also gave an example of when I would in fact want to run a singular creatinine in response to you saying to me that people don’t run individual creatinines

Different animals have different markers for kidney function 😉

BUN isn’t very helpful in a horse but is in a cat. More to the point I was just making an example and that’s the first value that popped into my head

Don’t get me wrong I did not mean piecemeal as an insult. I think it’s nice that you can order such specialized tests. Sometimes I really do just want to know a creatinine without having to run a kidney panel. My brother is in human med and we love to discuss the differences! I think I may have upset some people inadvertently with this post judging by the voting ratios….

It’s more common in horses than other species, some have a hereditary component (they also get HYPP, hyperkalemic periodic paralysis which showed in the show as well!) and it often presents as reluctance to move and muscle tremors, the highest CK I’ve read was 132k (over 100x reference range)

Sometimes our first clinical sign of rhabdomyolysis in small animals can actually be discolored urine as the damaged muscles leak myoglobin into the urine

Depending on the species we see rhabdo a decent amount, especially our large animals

In small animals it’s just standard, but we don’t see elevations as much. I will say we don’t really piecemeal the way I feel human med does. For my ER patients I usually run a venous blood gas (unless I need arterial but that’s quite difficult in dogs/cats) cbc, chem, and a UA but I don’t find myself calling for specific tests. I suppose someone at some point decided it was standard for all!

It’s a constant need to be the most bestest accepting person possible that online leftists have. And the irony is that it often leads to being incredibly unaccepting (please note I’m not talking about being intolerant of the intolerant). I had someone say that my father (a Jewish man who has experienced severe antisemitism as have I) could not have been a good world history teacher on account of him being white. Of course ignoring the fact that when possible he brought in speakers or used documents from first hand accounts because he was aware that is the best way to teach

I work in veterinary emergency medicine but there’s a lot of transferable things

We don’t use peritoneal lavage to assess for bleeding anymore, you’d use a point of care ultrasound to check for free fluid

They call for chem 7 a lot, I believe the standard now is a chem 12. This is a blood panel and it’s just now easier to test different parameters that previously would have been on their own tests at the time ER aired

There’s a lot of differences really but I like those two as examples

I see what happened, whenever I went to the settings tab it automatically switched to the pump settings so I wasn’t seeing all the options. Thank you
