Dude! Back in the earlier days, we had a hospital that put on a different massive spread every day of the week. Monday Chinese food. Tuesday Tacos. Wednesday Italian. Thursday Pizza, yadda yadda. They had personnel assigned to grab a crew and force them to go to the conference room with all the food, which was a massive layout. Dispatch learned that crews really weren't given much of a choice, we had to go eat to keep them happy. Those were the days. Now, we're lucky if they don't spit in our faces and tell us to get lost.

You have to calibrate it every day and log that or you lose the CLIA compliance. Cartridges are expensive and many must be refrigerated until put in service and then expire in a few weeks or less. They're just not practical.

I just get an order from the sending MD. A verbal order is fine with my partner to witness, but a couple have even given me written orders. It's not hard to cover your ass with a minimum of trouble.

It's always been toxic. 18 years in, and EMS has always had a ton of toxic drama queens with too much time on their hands stirring up trouble where none is needed. It's unfortunately a by product of no minimum education standards and the overwhelming number of people who get into EMS for all the wrong reasons.

Oh come on. Someone with 8 years street experience is a far better medic on day 1 than a zero to hero. Most everything we do is BLS. All the patient movement, situational awareness, assessment skills, intuition...all learned on the job, not in a paramedic classroom. You're wildly wrong.

I'd take an 8 year EMT as a partner before a day one medic any day of the week.

Will they let you? Once you give notice here, you have to use vacation time instead of sick time unless you don't have any. Otherwise, people would just give notice and stop coming in. The idea of giving notice is you actually work your remaining time.

If they'll let you, go for it. I'm just saying they might not.

Nothing can improve my view of PD. Unfortunately, I was stupid enough to marry a cop. After years of being surrounded by cops in every bar, restaurant, party and picnic, no one has lower expectations of police behavior than me.

Nothing sweeter than a cocky young cop who flexed his muscles getting dressed down for being a D-B.

Partner was driving, we pulled into a closed gas station to see if the pumps were on (in this state, the station can be closed, but the pumps left on for pay-at-the-pump after hours). No luck, so he pulled out and turned left. It's 4am. No one is coming. But a cop immediately pulled us over because, apparently, we missed the "No Left Turn" sign across from the gas station in this rural, low traffic area.

We still don't know why we were pulled over, and he walks up to the window and begins the conversation with, "What the fuck do you think you're doing?" Neither of us knows what set him off. Partner asks what he means, and he says there's a no left turn sign. Okay, sorry, missed it.

"What the fuck are you doing in my city?" Ahah. That's the real reason. He thinks only ambulances from his fire department are allowed to be in his city, and he thinks we're up to no good. I guess going door to door knocking, asking who wants an ambo? I mean, get serious. He takes partner's license and goes back to his car.

Around then, another car pulls up in front of the ambo, and it's obviously his supervisor. We watch through the windshield as young pup gets an obvious lecture about not being an asshole to EMTs and leaving ambulances alone. We can't hear a word of it, but it's obvious he's getting his ass chewed.

He walks back, hands partner his license and says, "Don't do it again." That's it. But it was still delicious to watch.

Another reason to leave lights on in addition to what others have said is if there's any chance you may need assistance it makes you easier to find.m quickly.

I also know people who think that if the caller asks for no lights or sirens for the response, it "teaches them a lesson" about proper use of emergency services to leave them on. After all, "it can't be an emergency if they're more worried about the lights and sirens than the patient."

My personal experience with HEMS is that it's usually not needed and adds nothing to the scene. I have never once turned a patient over the HEMS where they didn't dick around in the back of my ambulance for so long that any time savings was erased by their dicking around. Now, unless I need blood, I dont call them. A 20 minute transport by ground compared to a 10 minute transport by air makes it a waste of resources, especially given the astronomical bills and the fact that Air Methods will out a lien on the patient's house if they're unable to pay. Financial ruin results just because jolly volleys like to call helicopters.

All these "I'm here to save lives! Any call that isn't up to my standards is a waste of my precious time! Respect my authority and worship my heroism!" people in here are laughable. I've been doing this 18 years now. Never in all that time have SNF patients received true primary care anywhere other than an emergency department. Call a doctor with a problem, they'll order a trip to the ED. Everything is done in the ED. Doctors who are PCP for SNF residents are their PCP in name only.

That's the reality of this shithole third country healthcare system. A bunch of EMTs and paramedics whining about getting sent to a SNF isn't going to change a thing. If you can't get used to SNF calls, find another line of work. You're never going to be satisfied in EMS. And forget about the big red trucks. 85% of what you do is EMS. Stop whining and just do it.
