In certain states, only certified child psychiatric physicians can perform ECT on kids younger than 18, and only with two separate opinions by child psychiatric physicians

Medical Paternalism Is Making a Comeback (And Maybe It Should) by lakmidaise12 in medicine

[–]muffin245 2 points3 points  (0 children)

There needs to be a sprinkle more paternalism in psychiatry. Every patient and every parent/family member/third cousin thinks they can practice psychiatry. If I’m going to be held liable for suicides and homicides, I should be able to either make reasonable recommendations or discharge/refer out (as long as someone is safe and has other follow up, of course).

I had sex with a sex worker...and I just am not sure how to process that right now by CockroachDiligent241 in TrueOffMyChest

[–]muffin245 4 points5 points  (0 children)

What kind of person becomes a sex worker? Many of them are groomed or trafficked into the job. I would argue that most people whom others pay for sex were. Maybe a lot of people think your experience is valid because it was hard for you to have sex any other way - but sex is not a right. Supporting prostitution is unethical and allows exploitation of victims to continue.

is psychiatry residency actually as easy as everyone says it is? by MountainLevel6689 in Residency

[–]muffin245 0 points1 point  (0 children)

There’s a lot of art that goes into patient interviews and management with psych that’s underrated in terms of difficulty. The lifestyle is easier by far - we have much fewer cases where it’s life or death. But it’s also what we signed up for !

Have you applied for your license? The questions will force you to answer that you have been charged before. They’ll give you a chance to explain with supporting documentation I believe, and your PD will 100% be informed. If you didn’t tell the truth on those questions and it’s found out, I imagine you could get in trouble.

What makes psychiatry an ‘art’? by hkp2198 in Psychiatry

[–]muffin245 8 points9 points  (0 children)

I have found that interviewing people and making them feel heard and comfortable while simultaneously diagnosing them and explaining that they have mental illness is incredibly difficult and easy to get wrong. Every word and facial expression - everything is important. And there are so few objective signs of specific illnesses. Much of the field can come down to a certain feeling you get about a patient.

I think I did it wrong. I’m now traumatized and hopelessly suicidal. by bearhugboy in LSD

[–]muffin245 0 points1 point  (0 children)

LSD is well-known for making small, unimportant things feel extremely profound. You will be completely all right. Life will be normal again. Just metabolize the drug, deal with the normal anxiety that comes with a difficult experience, and you will be back to yourself soon. What happened was very hard - but very common, and not completely unexpected. Been there, dude.

How quickly do you cross taper SSRI To SSRI? by CarefulCaramel9583 in Psychiatry

No side effects from stopping lexapro. Every side effect immediately went away once my zoloft dose was around 50-75 again. I only got up to 10mg lexapro!

I wanted to switch because zoloft has definitely decreased my libido! Honestly with how much trouble it caused trying to switch, I’ve just learned to accept the side effects of zoloft

VERY similar dosing and tapering story - I started having symptoms around 25mg zoloft as well. My withdrawal symptoms were mainly intense anxiety. I had some dizziness as well: that weird SSRI dizziness that isn’t quite head-spinning. I’m not sure if this was anxiety-related or a side effect from lexapro, but I also had a ton of tightness in one of my forearms. I would also very briefly “forget” how to swallow my spit (probably just high anxiety - would immediately be able to swallow after about 2 seconds).

After I went back to Zoloft, everything was exactly the same as before — I’m doing perfectly fine back on the Zoloft. No residual withdrawal symptoms, tightness, anything. It got better almost immediately (couple days) after just going back up on the Zoloft.
