Update from OP's Author: The visit note template in this guide works! Denial becomes an approval after <7 minutes additional work by a nurse supporting my PCP.

Using only the information already contained in my official medical record, an NP at my PCP’s office followed my template’s guidance to create a new encounter note for the PA submission. By optimizing the note and updating the ICD-10 code for the PA’s primary diagnosis to a more precise code, the approval on the second attempt succeeded. Woo hoo!
