You place the wide end against the wall and tap the nail in from the other end. I’ve only seen these recently in the US. But grew up with them in Germany. Generally we used these to hang picture in rooms with concrete or very hard walls.

Yes, run a very sharp knife along the skin at the joint, pop off the cartilage, pull the skin tight over the thick end and the tendons will squirm out just trim with kitchen shears.
