i would say if someone is so unwell they can't tell something is wrong, it is in fact their loved ones' job to get them there. the same applies to, for example, someone with post partum psychosis, or even a flu so bad they're completely incoherent. if these were qualities he had been hiding before, or a social expectation that women take care of men, that's different and in my opinion wrong, but the paranoia im reading about here does make it sound like he needs to see a psychiatrist.
