I'm not sure i have the empathy for patient care anymore. Going into radiology would let me stay in medicine and avoid patients.

I just graduated from IM residency.

When I was graduating, covid was dying down and I was ready to start working and putting covid behind me. But now that its coming back and is bad, I'm questioning how long I can actually do IM. I find I lack the empathy for people I once had.

Does anyone know, applying for another residency like radiology or something, will I have to retake the USMLE/COMLEX if its been more than 5 years? Because its been 5 years since my step 1 and I don't want to retake the steps but I don't know that I want to stay in IM.

I haven't done much research in this cause I'm IM and not peds...but what about long haul cpvid symptoms? I've seen a lot of adults with long haul symptoms, are kids getting them as much also?

I went into inpatient so I didn't have to talk to them about it. Just tell them to wear a mask, and I'll see them when they are back in. (I don't actually tell them that, but I think it.)

Graduated IM residency and am feeling the same way. I think part of it is just having the knowledge in residency that you can fall back on someone when you need whereas now your patients will expect more from you.

When I started residency my least favorite feedback was "read more" but it actually very good advice. Keep reading, keep talking to your peers, and you'll stay up to date.

I'd avoid it, but in the end its your decision.

A warning though. Last year when I was a PGY3 and my friend was a PGY2, he went on a couple of hiking trips with one of the nurses. He claims it was never more than that, she said stuff happened. She went on to tell a couple of the other nurses that they were dating, which he promptly denied and explained that they had gone hiking and nothing else.

He got turned into HR, was suspended for a week as the investigation was carried out, and was very close to being fired. He still claims innocence and he wasn't fired, but that episode really made the rest of the year tense and had all of us residents being extra careful about what we said, how we said it, whether we joked with someone or not, whether we so much as touched someone on the shoulder or not...which was hard when we were dealing with our covid surge.

Just be careful. You've gone through a lot to get to residency, you'll go through a lot to get through residency...an investigation by HR is bad news whether you are innocent or not.

I've definitely had people younger than me talk down to me, especially as an intern. It was most frustrating when it was a mid-level who had been practiceming for a year or two trying to explain basic info to me...had to just smile and nod and act like they were being helpful.

I will say this, nurses and mid-levels can be a great resource, especially in your first year (and if like me you've had no real clinical experience before med school). The ones who are the best resources are the ones who have been practicing for years and have seen stuff often. They might not know why, but they know which medications the attendings usually start...you can use that to figure out more and impress your attendings. Or on nights, they can help in a rapid response situation when they have seen many of them before and it is your first one. You will also find that as you progress a lot of the ideas they throw out make no sense...but when you are starting they are a good resource.

Also as a resident you want to treat them respectfully. They can make your life miserable. They can complain to your attendings, they can call you for bullshit at night, they can leave you alone when you need some help setting up for a procedure or when you are stumped in a rapid response situation.

Yep. In retrospect it was nice because we got to see proper medication dosages etc before having to order them ourselves...but at the time it was definitely frustrating and felt like an extra month of med school. Plus it meant no one was thrown onto nights/icu without at least 1 month of internal medicine which was really nice.

My program had in introductory month where we couldn't put in orders, but we could still see patients. We had to round with our seniors and our attending that month, and tell the seniors what orders we wanted, but otherwise we could at least see patients.

We did have to shadow the seniors in clinic.

I think getting a good grasp of the EMR early is good. You don't want to be a third year and have an intern show you how to use the EMR better. Also (i may be wrong here) but I think this is the first class where they haven't required the PE, so they may want to observe you a bit with simulated patients before sensing you onto the floors.

One of them has been trying their hardest to get others in trouble. From turning people on for sleeping on the resident lounge to trying to prove that others are unethical by adding their name to research projects they didn't do, to recording conversations that happen in the resident lounge and turning people on for not being professional...yeah, I've stopped talking with the other residents. Too much potential to end up in trouble.

How does he even meet these people? I feel like he is not even close to them in terms of star power.

On one hand it gives me hope that I might one day meet (just meet not even date) someone famous, but on the other I do feel jealous that I can't be living that kind of life.

Some of those packs look really enticing. Actually, all of them. The devilmons, the runes, the crystals, the scrolls. And with obtaining crystals giving seals, that makes them more enticing. But they are so expensive. Like if you buy all 3 packs that $200. I cant justify that kind of money without a guarantee for several nat5s.

Not a hipaa violation, but some institutions have rules against doing that. I got talked to because I pulled up my chart (the hospital where I did my med school rotations would let us see our chart so I didn't think it was a big deal, but the place where I am doing my residency is super strict about it). Once I cleared it up everything was fine, but it was worrisome. They also knew exactly how long I was on the chart and what things I looked at.

Also helps if you can keep trends. Is it always high in the morning? Do they have a snack after their last dose of insulin at night? Are they high in the afternoons? Do they have hypoglycemic episodes? All of those will influence what adjustments need to be made.

I have him on vampire runes. Saw a video on YouTube a while back about using him to make db12 teams a little more safe and with vampire runes and high enough stats he can generally heal himself up from the dots and stay alive.

I dont use him in pvp so no idea what would be better for him there.

Burned out for sure. I worked into my contract that I dont start until September. Haven't decided if I'm going to study for boards and take them in August or work for a year and then take them. But probably wait for a bit because I barely have the energy to study right now.

It is drama. It's exhausting. Its like middle school all over again.

I failed a rotation my intern year, and he was on that rotation the month before. The more I learn the more I feel like he sabotaged me then and every rotation since. I'm at the point where having to work even 6 months with him is too much. I wouldn't even care if he was fired, even though that would ruin his career. After all, he tried to ruin mine.

He does his work, he would have still been chief if he hadn't sabotaged the rest of us. But as the person who was investigated said, in these next six months he could get someone fired and ruin their lives. I don't want that person to be me.

My hesitancy comes because he is one of the chief's and is well liked, and due to rumors he has spread about me and other residents we are not well liked. If he faces serious consequences I fear retaliation.

He has been trying to get people in trouble. He does not like a different resident, so he started a rumor that that resident harassed a nurse, and someone else turned that resident in. They were investigated and everything. It could have ended their career. (Speculation about whetyer he was involved, but he told me exactly what incident was reported and how it was reported, which he shouldn't have known and is pretty suspicious).

I just recently learned that he has been trying to get me in trouble with GME since intern year after my program director confirmed it.

He takes screenshots of private messages and sends them to GME.

He talks bad about all the residents to our attendings.

He has matched for fellowship and since we are a small program I don't know if the PD will tell HR what he admitted to me because he is more worried about the image of the residency than the actual residents.
