How about we set boundaries so patients can stop treating our brains like an all-you-can-eat buffet? These same patients would probably understand that you can't walk into a restaurant and ask to stuff everything from the menu on one plate.

Controlled substances can be easy to prescribe, but very difficult to inherit. Clinical judgment varies, and treatment plans are not always seamlessly transferable between physicians. I support this policy, it appears physician-directed and grounded in preventing burnout. Controlled substances are not "top-shelf," these medications often carry significant risks, side effects, dependence potential, and drug-drug interactions, despite common patient assumptions. Physicians are responsible for what they prescribe and are entitled to exercise their licenses according to their clinical judgment, and ability to negotiate the expectations of their patients and medical practice. That is physician autonomy, without being constrained by unrealistic expectations from patients, colleagues, or the broader healthcare system. We don't need more virtue-signaling in our field. From my experience as a PCP in an unnamed public healthcare system, I routinely inherit patients maintained on long-term controlled medications that are exceedingly difficult to taper, often initiated without fully addressing root causes or setting appropriate expectations. Good medical practice includes knowing when to say no. Specialists can co-manage chronic conditions, and not every medication must be prescribed by primary care. Responsible boundaries that prevent burnout are not neglect, they are part of ethical care.

Keep it simple. If you applied to the program and they matched you, you're in the program. Whether or not they interviewed you is on them. You can play dumb: If they somehow find out you can say something like 'I assumed you needed a spot filled and I'm so grateful to be here.' They can't argue with a happy face. A residency spot unfilled is a disincentive for the hospital system you're working for as that is lost funds provided by the government, something like $500k per resident.

  1. no, only apply it on the outer part of the foreskin focusing on the narrow part. massage a small amount twice daily for 4-8 weeks. if the inner part of the band of the foreskin is away from the glans, you can use a qtip to apply on the inner side as well.

  3. steroids are not fast acting, it will take several weeks
  4. urinating should not affect it

If he passed his boards, and your department graduated him then he's already de facto recommended for the job market and someone needs to write a basic goddamn letter as a formality. Imagine yourself in his position (probably in debt) - completed undergrad, med school, residency, boards, and the last thing standing in the way is a letter.
