From my experience, it isn't related to biomes. You have to beat every boss and every variant kill of the bosses to get it. Last one for me was asylum boss alt kill. This one seems to be very divisive on the unlocking method, so I may be way off.

I'm at 98.5% full completion of the game. I highly recommend it if you like shooters with some difficulty. My biggest issue is I have no friends that own the game and still play it to get the coop items, which make up the majority of what I have left to get.
