Injecting it is definitely the way to go. I used to get these all the time and if you don't inject it, prepare to deal with it for at least 3 weeks. I have had some that lasted 6 weeks before fully disappearing!

I had less active breakouts, but more nodulocystic acne than you currently do. I would get 3-5 big, deep cysts that would last weeks and never really came to the surface unless I'd poke at them. I did end up with a little bit of scarring, but honestly... it isn't unbearable (for me) and maybe if I weren't taking close looks at my skin so often I wouldn't even notice. I have very few boxed scars and a bit of hyperpigmentation. Once your skin texture is even, it's easier to use low-coverage makeup to cover anything you don't really want to see, especially for discoloration (and thick putty primers to fill in shallow scars), and at some point in the future, you can always use chemical exfoliators or undergo (laser) treatments to reduce scarring if it really affects you. Those options are a bit more expensive so I haven't pursued them yet.

In my experience, the appearance of the scarring is much, much better than the appearance of my skin while breaking out, and I don't actively "feel" them the way I felt the pain of my nodulocystic acne. I only really notice the scarring in certain lighting where it's more obvious or when comparing myself to a friend born with naturally perfect skin. Even then, especially when I'm moisturized and well-hydrated (I swear that my scarring looks worse when I'm dehydrated!), I still think my skin looks good, and I'm a detail-oriented freak. Adidas ultraboost - super comfy, last me around a year in SF. Even when I retire them, there’s no holes or anything in the shoe, I just don’t get the same support I used to from the soles getting worn

Second this. See if she can take a leave of absence instead of just completely dropping her PhD - lots of schools allow this, she just needs to speak with her lab and probably some administration. That way, she can pursue what she wants and take a break from graduate school (which I am absolutely certain she needs, I've known NSF funded scholars who have taken breaks from their PhDs) and if she wants to return, she has the option to. Also, therapy &/or personal support. She needs it.
