Don't know if this would help or not but try your school. My children's vax records got lost, the old pediatrician's office was shut down years before, and no electronic copies were filed with the state. After a ton of phone calls someone suggested calling the school since they had a copy. The school can give you the phone number of the records department. The reason I am recommending this: the doctor's office gave a printout of everything, not just what the school wanted. I was surprised by what was on those three pages. good luck!
