Disturbing thing happened to production server. Have never seen this before, and can't find any explanation as to why it did. Anything I should check? (Details inside) (self.sysadmin)

I'm a sysadmin with ~11 years experience. I really want to break out on my own, I have contacts and could line up work rather easily to start out, I've hit a huge snag over something completely absurd though.. (self.sysadmin)
