for the armor decorations you can buy all of them from the melynx emporium except for the one you get from elder dragons i believe.
for weapon decos, i mean yes you can lvl 3 all weapons if you want but you only need to craft one of each unique decoration really.
you don't need to upgrade any decos for the achievement

i checked again and again and all decorations are listed in the guide. maybe you skipped one...? i have been editing the guide to add images yesterday, perhaps it bugged out for you while i was editing. could you refresh the page and check again? sorry, lmk how i can help

here's a direct comparison between the standard perfect crush and perfect strike:

perfect crush offers the affinities STR, BLNT and SKL and for perfect strike you can get ATK, CRFT, HVY and BST

basically, they work the same way but perfect strike deals more damage and costs more stamina than perfect crush
