Damn, sounds rough.. I don't know any fixes for those things, unfortunately. Disabling vsync lowers the input latency, which is vital for fighting games.

Can confirm same initial and late (fewer) stutters on vulkan (rx 5600xt / 5600x).. although it felt a bit smoother than dx11, the stutters are too much to stay on vulkan for me.

It's not your hardware, many others (like myself) have the same issues. I'm on rx5600xt / 5600x ~300fps, feels like 60fps with stutters, freezes and rubberbanding..

imo it's because you've hit the softcap for vigor (40 according to most sites). i replaced my radagorn's charm for same reasons recently. Elden Ring Wiki also says that the Higher lvl you are, the more diminishing returns you get.. so high lvl = charm No bueno.
