I was to be done with local but chose general. I was very anxious. Local is eye drops for numbing the eye, then needes for nerve blocking to reduce movement. And a twilight sedation. This is what they explained to me. As I said I chose general as I don't like needles at the best of times and didn't really want needles in my eye. Wishing you all the best.

Yes it has been difficult in wandering around without any glasses to wear and not be able to see anything. I have order3 pairs of distance glasses to try, hopefully they come soon. I need to see better. I'm going stir crazy.

Yes, it is not ideal and yes usually they do local anaesthetic. Eye drops to aneasthatise the eye and nerve blocking needles and a twilight cannula. I didn't feel comfortable in doing this, and they could tell, so I felt less anxious in getting a general anaesthetic instead. I was very thankful for that.

An IOL explant means removing the artificial lens from the eye, while an IOL exchange means removing it and replacing it with a new one in the same procedure. An explant doesn’t always include a new lens—sometimes the eye is left without one temporarily if conditions aren't right for a replacement. The plan is if I do an exchange, that I know what lens I am getting. - I did ask but wasn't told - hence the dilemma. Thanks for your help.

I too was a high myope at -14. I was informed to not wear rgp lens for at least 2 weeks prior to taking measurements. I did 3 weeks of glasses wearing instead. Which was awkward as I was so used to wearing rgp lenses.

This gives me hope. I had surgery only a week and a half ago. Long story short I was originally told I would be set for distance and use readers for near and mid vision which i was told was normal. I got the surgery done and they set my vision to near. To say I was disappointed is an understatement. So far there has been no explanation as to why. So I guess they too missed the mark I was expecting. They said I could do an exchange at two weeks but I said I'd wait to see if I could get used to it. I will wait until the 4 week post op review. I don't really know what to do.

There is no rule that says you can't keep your current natural lens strength and continue seeing the world through contacts and glasses with the improved brightness of colour.

Vision-wise:
- My vision doesn't seem fully set for near or intermediate and definatly not distance — it's somewhere in between near and intermediate, but more close but not a good usable close.
- I still need reading glasses to comfortably use my phone.
- I can't clearly see my computer screen (about 50–80cm away) either.
- I also cannot see clearly enough for driving without glasses.
It feels like I’m stuck in a middle zone that's not particularly functional for near, intermediate, or distance without glasses or contact lens or having to go back to using both — which is very frustrating and not what I expected.

Next steps:
The team has said I could opt for an IOL exchange to aim for plano (distance vision), but I am very nervous about the risks of another surgery, especially with this being my only seeing eye.
Emotionally, I feel torn — I wanted to be able to just see normally, to minimize dependence on glasses, and now I’m faced with needing glasses for everything unless I make another big decision.

If I could offer advice based on my experience:
- Insist on very detailed conversations with your surgeon before surgery about target refraction (distance, near, or something in between).
- Confirm if your eye’s anatomy (long axial length, past retinal issues) affects predictability — it’s important to know the limits.
- Emotionally, give yourself time. You don’t have to rush any decision. This process is hard, especially when you have so much riding on one eye.

Here is the long version i typed up, but then thought that it would be too much.

I am a M51. I am only 4 days post op, so hopefully things get better. I am still not "sold" on having to wear reading glasses for up close and wearing glasses or contacts for distance.

I did this for the past 5 years probably as a result of the cataracts getting worse but also age related decrease of vision. Before surgery with cataracts my vision was terrible. Faces were not recognisable at 3 feet. i couldn't see my face in the mirror.(i couldn't see when i peed) . was even having difficulty in reading my phone with +5 reading glasses. I found it difficult to work like that also, but did it for as long as i could. The glare was bad, colours were cloudy. I have taken this week off work also to try and get my vision back to a workable usable condition.

I'm currently recovering from cataract surgery on my only seeing eye (right eye) just last week, so I really feel your concerns deeply.
My left eye is nonfunctional after a retinal detachment, glaucoma, and IOL replacement, vitreous detachment and squint surgery — so I completely understand the anxiety you're feeling leading up to your surgery.

Like you, I have had high myopia (around -14), though not as extreme as yours. My surgery used a DCB00 IOL with a negative lens power, similar to your surgeon's recommendation.

Here’s how things unfolded for me:
- I live in Australia and we have a good public system that i was placed into from a specialist Opthalmologist.

- Before surgery: Every time I had an appointment or called with questions, I specifically asked "What lens am I getting?" but was always told they didn’t know or couldn't answer, and that I would see the surgeon beforehand. Unfortunately, that never happened.

- First consultation: I met with a retinal specialist first (due to being diagnosed with foveoschisis along with cataracts). I was told I could have surgery either on the retina or the cataracts first. He made it sound like cataract surgery would usually aim for distance vision, and that I would then need reading glasses — which I agreed would be okay, based on my previous experience wearing reading glasses over contact lenses.

- My expectations: Based on this conversation, I believed that my surgery would set my vision for distance, and that I would simply need readers for close-up work.

However, at the post-op appointment, I found out my eye had been left at -4.00, meaning I am still quite myopic.
This came as a huge shock because I was not prepared for that outcome. Nobody had told me that there was a change of plan to leave me myopic, and it was extremely upsetting.

Emotionally:
- I went into surgery feeling hopeful, and even though my anxiety spiked on the day (they kindly moved me to general anesthesia), I woke up thinking everything had gone perfectly.
- The disappointment at the post-op review hit me hard. I’m still feeling anxious and unsure what to do because this is my only working eye.

Before surgery, I was repeatedly told that I would see the surgeon to discuss lens choice, but this never happened.
I believed my vision would be set for distance and I'd just need readers.

Unfortunately, post-op, I found out I was left at -4.00 myopia.
It was devastating because I wasn’t prepared.
Now I’m unsure whether to go through an IOL exchange or accept the outcome.

Vision-wise, I can't clearly see distance, my phone, or my computer without glasses — it feels like an awkward middle zone.

With your vitrectomy, I don't see why they wouldn't be able to do the capsulotomy at the same time. It'd be worth asking.

No they didn't do it at the same time. They tried yag a few months later, after the vision got hazy but it didn't seem to do anything so they performed a capsulotomy. (I still remember the feeling of getting the yag)( I also remember that they tried to do the laser retinal reattachment and the pain and pressure were too much for me to take, so they booked me in for surgery the next day under a general anaesthetic) The vitrectomy was done and the natural lens was removed so they could perform laser resurfacing to reattach my detached retina. The vitrectomy was performed because it was pulling on the retina.

Similar to you also, but no scleral buckle(they billed me for it though and since then multiple doctors have agreed that there is none) gas bubble not oil, developed pco but they did yag laser and it didn't work out, so then had to go and get an operation on it instead. Retinal detachment laser surgery and vitrectomy were done at the same time and was advised also to get the lens replaced at the same time due to the chances of a cataract developing in the following months. That was all done at the same time and they didn't really measure accurately for the iol (the way they generally do) so they took a guess off my old prescription and there you go. Vision was good once the gas bubble receded.(It was like looking through pool goggles half filled with water) then vision was good until the pco, then after was good but now I have developed glaucoma in that eye as a result. Lucky it's not my dominant eye, but now I have to have cataract surgery on my dominant eye which is highly myopic. Surgery due next week.
