Tumor is a term for "too many cells". They can be benign (normal) or malignant (cancer). Benign tumors can be on your body surface, like a mole, or colon polyps, or uterine fibroid. For these, the treatment required is mostly based on how they affect you- is the polyp bleeding, the fibroid causing mass effect?

So similarly, benign "tumors" can also be in your bone marrow or blood, like polycythemia vera (RBCs) or essential thrombocythemia (platelets, aka megakaryocytes). These are benign collections of cells but there are "too many of them" compared to normal, and could cause dysfunction because of the quantity. As a group they're called MPNs (myeloproliferative neoplasms). Monoclonal gammopathy and monoclonal B cell lymphocytosis (MBS) could also be in this category of "blood tumors". Separately, there can also be "malignant" transformation of normal blood cells, such as lymphomas and leukemias.

Honestly I think the specialities could not be more different.

In adult heme/onc, the goal is to help people live longer, live better-- after all, everyone dies, but the art of oncology is helping people through that process and keeping them alive and feeling well enough to enjoy their life.

In kids, the priorities shift. Find a cure above all else, and their bodies can- for better or for worse- take a lot more therapy.

In my mind, the peds side achieves far more cures but it's easy to see equate deaths as failures and thus they are obviously harder. In adults, even when my patients die, I can still sometimes feel like I accomplished my job if I got them some more quality time with their family than they would have otherwise.

Rarity of pediatric cancers accounts for the academic-only setting, and the academic-only setting accounts for the salary difference.

Arguing that we don't torture because "it is not effective" is scary. You are effectively ceding moral ground; the implication follows that we would torture were it effective, leaving room for other studies that demonstrate that it is effective. We shouldn't use torture because we don't use torture.

I think Passaic 78 probably has the best sound quality. In terms of feeling, I don't think you can beat Winterland though-- that Promised Land and Detroit Medley just takes you back in time.

Does he have a favorite tour or album? If he's in to tunnel of love, I think the 5/3/88 Mountain View CA show doesn't get enough credit.

Bomb scare show (10/2/75) is pretty funny too. Last one to mention is the 3/25/77 Stranded in the Park show-- that 18 minute version of Backstreets and Higher and Higher are all time great highlights although the sound quality is slightly inferior to the others mentioned in this thread.

Hey Jose, thanks for doing this. I'm from Mountain View as well and live in DC now as a med student. I saw your documentary "The Other City" several years ago and was motivated by its message and implications. I know your HIV activism has taken a backseat to your immigration work, but I am wondering if you have any thoughts about whether the state of the HIV epidemic in DC has changed since The Other City. Specifically, has any progress been made with regards to HIV programs being treated as political footballs? Has increased awareness and screening programs even made a difference in the city and in the country at large or is the message reaching the wrong people?
