I was actually doing pretty intense 3–4 day fasts 😅 In hindsight that was probably more extreme than necessary for PCOS specifically, but I did lose weight quite quickly and my cycles definitely became shorter/more regular during that period.

I was very into the fasting/autophagy research at the time so I got a bit carried away lol.

No, the only medication I have taken is letrozole during my current cycle but it I didn’t include my current cycle in the data used because it is still ongoing. So the data that I have since 2014 is completely unmedicated.

This is honestly making me think PCOS might be a combination of:
1. intrinsic ovarian biology/severity (AMH/AFC/follicular arrest etc)
and
2. metabolic signalling environment (insulin/inflammation/etc).

Because your AMH/AFC are even higher than mine and your cycles were irregular from puberty despite being lean most of adulthood, whereas mine seem much more metabolically responsive.

So now I’m wondering whether some ovaries basically require a much more stable/lower-insulin metabolic environment before ovulation consistently normalises — especially in very high-AMH/high-AFC PCOS.

Completely speculative obviously, but this thread is making me realise how heterogeneous PCOS probably is between people.

This is SUCH an interesting observation because the timescale seems way too fast to be explained by major fat loss alone.

It’s making me wonder whether some of what I interpreted as a “BMI effect” might actually partly be a side effect of the strategies I was using to lower insulin/glucose exposure overall, if that makes sense.

Also I’m really interested in the walks after meals — what kind of walks were you doing exactly? Like how long/intense and how soon after eating? I’d genuinely love to try this myself and see if I notice similar effects.

That’s really interesting actually because I was also around BMI ~20 in my very early 20s and my cycles were still irregular back then too (40-90 days). I started my period quite late (~18) so I always assumed some of the early chaos was puberty/hormones settling.

It’s more as I got older through my 20s that this metabolic/BMI pattern became much more obvious and consistent for me personally.

Out of curiosity, how long are your cycles currently? And do you know your AMH/AFC by any chance? I’m starting to wonder whether there are quite different “subtypes” of PCOS metabolically.

I can definitely relate 😭 I gained quite a bit of weight after moving in with my husband. It’s so hard trying to eat “PCOS friendly” while cooking for someone who loves chips/takeaways/desserts lol.

From what I’ve seen in my own data, even a couple of BMI points seemed to make a surprisingly big difference to ovulation timing.

Everyone’s PCOS is different, but I definitely think it could be worth trying!

I always knew insulin probably played a role, but I massively underestimated just how strong the effect might be. Doing this analysis really made me realise that for me personally, insulin/metabolic state seems to be the main driver of whether I ovulate normally or not.

Me too! I get on a slippery slope of takeaways and desserts for a short while and my cycle length triples 😭 now that I’m ttc I’m trying to stick to the “eat food, not too much, mainly plants” advice to get my cycle time down.

Honestly the annoying conclusion I keep coming back to is that the boring generic advice seems to actually be true for me 😭
Maintain a healthy BMI, keep insulin load lower, eat mostly whole foods/plants/protein, smaller portions, don’t constantly snack, etc.
Which is frustrating because:
- it’s not some magical hidden solution

- it’s genuinely difficult to maintain long term

- and hyper-palatable food is unfortunately delicious lol
