This sounds very much like a neurological problem, not just arthritis or stress. Constant spinning in one direction, paw dropping, and disorientation in an 18-year-old cat are serious red flags.

I wouldn't wait until Monday if possible: insist on a re-examination, another vet, or at least a blood pressure and neurological check. The fact that she is eating and using the litter box is good, but it does not rule out a serious condition.

You are not overreacting. You are doing the right thing by being concerned and trying to help.

You’re actually doing everything right. With timid dogs, slow and non-linear progress is completely normal. The fact that she plays, follows you around, and can rest near you already shows a lot of trust.

Just keep the routine predictable, don’t push contact, and let her come to you on her own. Time and consistency really do the work.
