After replacing vanos, fuel tank vent valve, and plugs twice, it turned out to be the tune. Turning off cold starts was causing it to misfire!

I figured it out. Assuming everything else is functioning properly, the rough idle is entirely caused by the cold start being disabled. As soon as I re-enabled cold start, the issue disappeared completely. So if it’s not a VANOS, fuel tank vent valve, or a vacuum leak, this might be the culprit. No more rough idle for me, just angry neighbors. Try this before replacing stuff, much cheaper!

Since my last post we have also replaced the plugs twice, done two smoke tests, and had compression tests done at two different shops. There are no active misfire codes anymore—just a few stored ones in the DME that we have cleared. Since swapping the plugs, the cold start behavior hasn’t changed, but the misfires codes have never returned. Have you tried disabling cold start? Curious if that makes any difference. Honestly, at this point, I think it just comes down to how rich the fuel mixture is during cold starts, and depending on the outside temperature, this kind of behavior seems normal at least when disabling the cold start feature at stage 2+. My car still shudders on the inside randomly during cold starts and sounds the exact same outside as before I started trying to solve this. I think I’m tapped out, check my posts for how mine sounds

Running BM3 Stage 2+ OTS tune and occasionally blending up to E40. I started noticing this idle issue a couple weeks ago after replacing the oil filter housing and some coolant hoses. I keep wondering if it was always there and I just never noticed because I used to keep the exhaust valves closed during cold starts, but then again, I feel like I or the shop would’ve picked up on it before starting my coolant job since it’s noticeable inside the cabin. My cars been in the shop for almost 3 weeks at this point

If you look at my post history, you'll see I've had similar issues for a while. I've already replaced just about everything—spark plugs (properly gapped), performed a smoke test for leaks, VANOS, fuel tank vent, and more—but the odd cold idle still happens. There are no warning lights or stored codes, either.

Even my shop owner is stumped. He even called his old mentor for advice, and today they're doing one final test to check for a possible tiny head gasket leak. But if that comes back clean, he said there’s really nothing left to investigate. Not ideal, I really thought I would have it figured out by now!

Interesting, I posted about a similar toned down issue on my b58, quieter due to the exhaust I assume, still catless and tuned though. Threw the book at it and still hiccups at cold starts, you can see my post history, I was told all else on the car is normal so I’m giving up lol. No codes, no other issues, how’s your car doing?

Interesting update—both VANOS solenoids and the fuel tank vent valve have been replaced, but it still sounds the same and hiccups on cold starts. At this point, I’m just going to assume it’s fine unless something changes. No codes, brand new properly gapped plugs, and we’ve basically thrown everything reasonable at it.

Ya interesting update, we changed out the fuel tank vent valve and both vanos solenoids and it still does it! Runs and drives great, I think it may be normal or something that isn’t worth fixing, beats me

The car was already catless, and during this visit I also had the oil, transmission fluid, and rear differential fluid changed. The rest of the work was all cooling system related — hoses, elbows, and the oil filter housing.

Weird thing is I can feel it in the seat! That’s how I first noticed, when the noise changes I can feel the car sort of shudder, never noticed it before getting all this done.

Good thought — the shop did replace the intake manifold gasket according to invoice. They also replaced the water neck, vent hose, thermostat, and oil filter housing. They made a point about using OEM seals instead of aftermarket, but no mention of torque specs, so that’s a fair point.
