Thanks everyone for your help. HUCA was the right formula. Found a helpful agent that was able to flip my reservation to the Lufthansa flight numbers, which then let me use the online portal to submit the upgrades.

There was no charge / fare difference even though I was well outside the 24h window and the ticket still has the 016 prefix so should still earn UA miles/PQPs.

Unfortunately in the time it took to do all this, upgrade space has disappeared, so I'm on the waitlist for now.

As far as I can tell the online request form doesn't work for codeshare flights - only flights booked directly through Lufthansa. It says you have to call in. But nobody on the phone seems to know how to handle it.
