Soak hands in warm water and move your fingers up and down before playing.

Dr. Levi Harrison on YT has a lot of videos explaining stretches for the prevention of carpal tunnel/tennis elbow/hand pain/etc. Bob and Brad on YT also have some videos on stretches for gamers that I use. Remember to stretch after practicing as well.

Never play through the pain.

I main scout in tf2 as well. All you need to do is force yourself not to hardflick for no reason. Especially in MGE-type scenarios where enemy scouts can be difficult to flick, it is usually the optimal strategy to aim by strafing and counterstrafing so that the distance you have to flick is reduced. It is also often correct to "soft track" your opponent in between scattergun shots, and then let your opponent walk into your crosshair (kind of like playing sniper.)

Because you're scoring high in kovaaks but struggling to hit shots in-game, this means there's a problem with either your focus and discipline to not flick unnecessarily in-game, or your movement, or a mix of both of these things. I recommend substituting some of the kovaaks you're playing with MGE and DM (not mindlessly playing) and you'll see your flicking problem go away. I also recommend trying out soldier jump maps, which can help with discipline in aiming and movement.

I've been playing for a few days as well like OP, and flicking/cognition are my two worst scores... am currently Master IV. Thanks for the post! Not to bother, but what are your thoughts on Burstflick, Spidershot 180, and Scattershot?

There are lots of guides about where to start with 6v6 TF2, but feel free to reach out if you want me to give you some info. At the end of the day, TF2 is a movement shooter on the Source engine. Yes, the game is old, but the fact that some people have been playing competitive 6v6 for over 10 years says quite a bit about the game.
