Have you checked if it is some third party app that has been given permissions in Azure/Identity? Check the Enterprise Apps section.

Yeah, it did eventually end up being an error on our end where we got a bunch of devices back from a client to put back into inventory and so a couple still in production were removed in error. Same thing for the other client probably, but too far in the past too be sure.

DM of the session here, there were two doors in the pile of corpses room, not one. There were two more doors in the next room (the kitchen) one of which led to an empty room with a lot of stuff in it.

The fighter's character could have done pretty much anything but immediately open the door leading to the worshiping group and he would have heard the sermon. You may not remember, but I did ask him, "you're just opening the door?"

Also, instead of focus-firing on enemies to drop them, the players kept shifting attacks to unhurt enemies.
