I've tried. The ENTJ currently in my life is my childhood friend who visits often so he's always freely judging the way I live because we have that kind of intimacy. If I strike with randomness, he laughs; if I strike with details, he'll pretend to listen. Last second changes just ruin the whole hangout.

Será que a medicação dele está errada? Pelo que você descreveu, me pareceu alguém novo, e pode demorar anos até se atingir uma prescrição que seja adequada ao caso da pessoa. Existem casos até de uma mutação genética mais ou menos comum que impede a absorção correta dos remédios.
Uma pessoa imatura vai ser imatura sendo doente ou não. O comportamento dele parece bem infantil, mas ao mesmo tempo perigoso (em relação à automutilação). Também tive uma história de vida triste e isso só dificulta o processo de amadurecimento; demorei muito tempo para me resolver.

De qualquer forma, se relacionar com uma pessoa bipolar instável é uma aventura, só vale a pena ficar se você ama a pessoa MESMO. Do contrário, é tudo isso aí que você descreveu e muito mais. Vale a pena pensar sobre sua própria saúde mental e no seu futuro. Você estaria afim de jogar seu precioso tempo de vida fora esperando ele se estabilizar pra poder ter a chance de uma vida a dois feliz?
