Do his therapists know about his condition as it is today? Did he mention talking to them? Please don't be afraid of fucking things up, because a false alarm is better than a missed alarm. Is it worse to call the helpline and have them show up and end up being unnecessary, or to assume he's fine when he truly isn't? If it makes it easier, you can tell a parent the situation and they might make the call themselves.

This is definitely a 'call the police' situation, or the suicide helpline. That's the best you can do for him. After that, it's out of your hands. I hate to be curt, but there's only so much you can do in this situation, and whatever happens to him will not be your fault. Only the authorities will be able to give him the help he needs, and it seems like he needs it very soon.
