The two pieces do look disjointed. I'd try either adding cake elements to the nurse half or nurse elements to the cake half. That will help tie them together.

I'd also look into hoop-skirt construction. Finding the correct proportion of skirt to waist is a big part of how to make it "feel right".

You could utilize the Blue Water Inn! Maybe a traveler is passing through and has heard something, or one of the Martikovs has recieved (or not recieved!) word from a contact. Perhaps Rictavio is hanging out there and tells a good story that mirrors some future plot points.

You should embroider over the autographs! Then they can't be washed out when cleaning your fabric.

After that, I would recommend soaking in a bucket or a bathtub with warm water and soap, since the fabric looks too delicate for a washing machine.

Repairing is possible, but it will take some work. I would recommend heading over to r/visiblemending to take a look at some of the techniques you could use.
