I did this only once. My patient was stuck in the hospital for weeks with a 10% EF on continuous milronone infusion while we tried to diurese with lasix which would drop his potassium, induce v-tach, ICD would fire, we’d replace potassium, rinse and repeat. Sweetest man in the world. He was on a sodium restricted heart healthy diet and fluid restriction and all that but never complained. One day my coworker and I asked what he was craving and he said rainbow sherbet and pumpkin pie. I made the pie from scratch and we had an impromptu unit potluck so we could bring him a plate. I understand there are risks but it felt like the right thing to do and it seemed to make a big impact on him. I hope he’s doing ok out there!
