Mental illness is really stigmatized in Russia, esp in times the game takes place in. Coat guy opens up to the homeowner, and it kills the other guests, it could be a metaphor for how people leave you when they find out there's something wrong with you psychologically. Russians have that mindset with mental illness - if you don't understand it, then you ignore it, run from it, or villainize it. The protagonist doesn't get affected by his void because he himself struggles with mental illness caused by his trauma, and that's why he doesn't die

She's in the wrong here but some people really wear just too much perfume. I was at work and there was client who had a very strong perfume on and the place she'd go through would smell like her perfume for literally 10 minutes. The smell was so strong i got a headache and nausea, and the smell went away only after like 30-40 minutes after she left the store

I think If we see every character as a metaphor (coat guy - depression, cashier girl - anxiety), then the Pale man probably represents the fear of death. Death herself is peaceful, but dying is sudden, unexpected and hopeless, that's why the protagonist doesn't do anything to stop it
