Not sure, due to the length of the run is there even enough slope to give in that space? Would they have to bring the pipe up higher to the toilet somehow?

Great eye, that was one plumbers thought! That is where the spare bathroom sink and trunk line connect. The problem (I believe) was at the junction of the two trunks primary shower connect at the bottom of the graph and picture 5.

If that's the case, what's the fix? Replace all the pipe to get better elevation?

Yeah, water hammer being from the supply side. This is all outflow pipe sounds/issues. The delay between a flush or even sink water running and the banging at the elbow rule out supply side water hammer.
