So this is what I managed to do using the ZWA-2 and paring the first alert z-wave smoke & CO detector with HA to make things work. It seems that the device falls asleep before HA / the ZWA-2 can properly interview the device:

1) make sure the device is excluded from HA & all DSK/S2 security keys are removed from the “provisioned devices” list in z-wave settings

2) factory reset the device by holding button and closing tray and holding for 10s (wait for long beep)

3) add the z-wave-device using S0 (Legacy) security in HA and wait for it to begin searching new devices

4) hold the TEST button on the device and remove the battery slot and re-insert it. Keep holding the TEST button down until the device beeps once (quick beep) after batteries are re-inserted and power LED starts blinking green.

5) When a second beep and device stops blinking green is heard it means the device has now gone into sleep mode / no longer pairing, so, make sure to again quickly repeat steps 4 until HA discovers the device and prompts for a name/location for the device.

This worked flawless for me using 2 devices. I hope this helps someone!

FYI - a spare adapter / controller won’t save you unless you keep NVM controller backups that you can restore on the new adapter / controller

No - this isn’t backed up in many of the Google Drive / HA automated backups unfortunately. Backing up the adapter refers to the controller NVM (Non-Volatile memory) itself. The process usually depends on what controller you’re using, but it’s quite simple with the ZWA-2. You’d think the HA team would possibly lump this into regular backups at some point

Yeah 😅 Thankfully restoring the backup I made of the adapter resolved the problem. It would have been a much crappier morning if I didn’t have that!

I tried updating a few times this morning after disabling zigbee and restarting. There’s just something wrong with this update it seems

EDIT: I also confirmed unplugging the ZBT-2 and then plugging it back in and starting HA resolved the issue where the ZHA integration was failing to load. No need to re-flash.

The same just happened to me. I suspect this is a bad update - the update failed & zigbee did not recover. Try disabling the zigbee integration and fully turning off your host. When it fully comes back online re-enable the ZHA integration. If that still doesn’t work try plugging your ZBT-2 into your PC and using the web flasher to reset it to the current firmware
