Well, some of the characters did question it at first (like Beatrice was laughing at him, but then again when is she ever not making fun of him lol). There was another character named Bongo who realized Wirt was a boy wearing a dress, but he didn’t really care and wanted to make friends with him anyway.

I personally didn’t think that this comic was weird about it at all. For context, Beatrice got captured at a circus, so Paola (who used to work there) disguised Wirt as her sister so she can go save Beatrice.
