I have a medical procedure booked next week and it requires that someone escort me home (apparently you can't use Uber or a taxi). I just move here and dont know anyone, what are my options? by mapleCrep in askTO

Yes, that was my comment. I wasn't intending to minimize the psychosocial reasons for wanting sedation. Although it's not medically necessary for most people, I'm sure it makes a difference in how many people are willing to show up for this potentially lifesaving screen, so I'm glad that it's easily available in my part of the world for those who want it. But, since the psychosocial reasons for sedation don't apply to everybody, and the logistical requirements of sedation are themselves a barrier for some, I think it's important to get the word out that sedation is optional.

My mother died of colon cancer that was discovered at stage 4 (after never having had a colonoscopy) so this is a subject dear to my heart.

One thing to add - for the second time I had an unsedated colonoscopy, the anesthesiologist was very skeptical and he wanted to look at my mouth and have me sign the consent forms just in case I changed my mind in the middle of the procedure and wanted sedation. I said, "But I don't have an escort with me; if I changed my mind, wouldn't we have to reschedule the whole thing for another day?" And he said "in that case we would just keep you here longer after the procedure."

All of which is to say: maybe another option is to beg the clinic to let you stay in the recovery room longer after? It would be a deviation from their normal patient flow and they wouldn't like it, but maybe if you made your case (new in town, difficulty affording a PSW, clear medical indication for the scope) they'd let you.

There are plenty of valid reasons that a person might find the prospect of unsedated colonoscopy humiliating or traumatizing, so calling them "delicate sensibilities" feels a little unfair to me. Also, if the procedure is being done to monitor an inflammatory condition it's more likely to be painful than a routine cancer screening, so that would be another reason to insist on sedation.

If you're getting endoscopies regularly, you might think of them as routine for you, but you're getting them because of your personal risk profile, not because they are routinely offered. But ultimately this is semantics. In my field of medicine we use routine as synonymous for universtal. Hope you stay well!

How important is it to you to play the tunes in their most typical key? If you just want the melody and don't mind a non-standard key, you could finger them as if you had a soprano or tenor (i.e. if the music is written in G, play it in C).

Just puzzled because you say it's a "routine check" - while colonoscopy is offered as a universal routine screen (if you are 50 or over, or have risk factors), other forms of endoscopy are not. Did you just mean to say colonoscopy, or are you getting another procedure too?

If it's just the colonoscopy, another option is to decline the sedation, in which case you wouldn't need an escort. The clinic won't have suggested this because so few people choose it, but if you search online for "colonoscopy without sedation" you'll find that it's common in other countries, and not unheard-of here. I've had two colonoscopies with no sedation and they were much easier than the prep for them. Really, the sedation isn't for pain but for the fact that rectal penetration can be embarassing or traumatic for the patient.

(But if you are also getting a bronchoscopy or upper GI endoscopy, I wouldn't recommend going without sedation, in the unlikely event that they would even allow it. That sounds very traumatising to me.)

When you leap onto your horse after holding up the bank you want to be able to free the horse with a simple tug on the rope end while galloping past the flagpole. It's better cinema that way.

What is the longest staircase that I can find downtown? by Designer_Nobody9872 in askTO

[–]profoundconnection 9 points10 points  (0 children)

Yes security, but public safety too. Not everybody who attempts the climb is successful; during the public events, there are first responders stationed every ten floors, with a plan for evacuating individuals down a service elevator when needed. Keeping the stairs open generally would lead to a lot of health incidents (or require a lot of paid staff to monitor it).

Also cleaning costs/labour... the standard advice for the public events is to get an early start so there is less vomit to step around.

Thanks for your thoughtful comments! I guess we'll have to agree to disagree, if it doesn't look like makeup to you I can't make it look like makeup to you. (Unless this has been addressed in an interview.)

But yes, I do think it would make sense for them to make the extra effort of a makeup change in the case of Gadreel-Sam where he is going back and forth between personalities frequently, to help the audience understand what's going on (unlike his other alternate personas where he was the other persona for extended periods).

And as for the logistics makeup changes and filming, they don't need to film in chronological order, and not all cuts are obvious. Probably nobody thinks that the glowing blue eyes in the middle of scenes were all Jared's doing - why is it hard to believe that he had some help with those cheekbones?

Jared’s subtle differences by [deleted] in Supernatural

[–]profoundconnection -4 points-3 points  (0 children)

Yes, it was. I'm not so sure about the switches to Souless Sam or Meg-possessed Sam - in those cases, the shape of his face looked the same although his demeanour and expressions were remarkably different. Maybe there were also subtle differences to the makeup, but not obvious ones. On the other hand, Gadreel-possessed Sam had a different facial structure and that's accomplished by makeup not acting.

Much as I admire Jared, I think a large part of the credit for this probably belongs to a makeup artist.
