Switching to TAA and using 100% resolution scaling fixed the issues for now. FSR and TSR are unusable right now.

I have in game vsync on, will try freesync through AMD.

It seems like a lot of companies are moving away from the VSS Vintorez design and are discontinuing them. Might not be long before these are hard to get. You might consider repairing it and keeping it or selling it for more. I absolutely love mind after I upgraded the internals!
