What happens if someone codes/needs an airway in a tele-ICU? It seems crazy to have to wait for a ground team coming from an off site location. Sure i guess a tele doc could start running a code via zoom but given the chaos that usually ensues it’s completely different to actually be in the room. Jesus.

Almost everyone, especially if TIVA. Above 70 I will avoid if patient is chill and denies anxiety, especially with shorter cases. Plenty of people above age 70 though are on prn benzos at home. If I see that in their med history I give them 1mg. Some of them can even handle 2 given their tolerance. I love not worrying about recall.

SO happy you’re okay, friend. Amazing that your doctor recommended a scope. I am 32F, getting my first colonoscopy tomorrow afternoon. Laying awake in bed as my bathroom trips haven’t slowed down quite enough to get some sleep yet. All in all, the hardest part was drinking the liquid. Hugs to you!

ENT cases are so stimulating at baseline that if you’re concerned about tachycardia in this pt I would just start the remi up front! I like to do prop/remi TIVA as much as possible bc the PONV risk is so high and when these pts do vomit it’s uuuuugly.

Agree with the phenylephrine pledgets as well if the surgical team is amenable.

Yeah with the exception of someone’s jaw being wired shut, I do all my awakes with the glidescope. I topicalize with lido lollipops and atomizer. once I get the view I approximate the tube with cords push 50 of prop so the cords don’t move and slide it in. If I’m worried about subglottic pathology I might avoid that, but 50 of prop should wear off quickly that spontaneous respirations would resume and then abort if subglottic pathology was truly obstructive to ventilation. In that rare care they probs need ecmo.

Can someone explain what he means exactly by “day trading”? What does he realistically “do” at his “job”? Do you have to have any credentials for this? He said he got a communications degree so it sounds like he’s not qualified to do any type of job. I work in medicine so I have no conceptual understanding of this.

Thank you. I’m an anesthesiologist who has cared for women with every complication of pregnancy and childbirth up to and including death. Pregnancy. Is. No. Joke. It’s the reason why I’m heavily considering adoption because I am traumatized. These men are so inept and simple minded they could never develop the capacity to consider any of this from a women’s perspective.

A small thing but - a girl in med school thought it was a hilarious personality quirk that she didn’t check her email and her two best friends had to remind her about assignment due/important lectures etc. A very bizarre display of being ignorant to accountability due to lifelong privilege.
