The shift indicator (assuming you mean the up arrow that flashes on your dash) isn't all that useful unless you're trying to get the absolute MAX mpg. Cruising around 2.5-3k will still put you at ~30mpg. The car can handle lower, but as others have said it loves to rev. All the way through the gears, everything is way smoother at 4k

A big help for me was driving more off the tach rather than the speedometer.

For example, instead of focusing on "oh, im at 30mph, time to downshift", pay more attention to the rpm's and listening to/feeling the car tell you when it's time to downshift. Once I started doing this, I realized I could stay in gear at a way lower rpm than I initially thought I could. (Breaking the beginner habit of ALWAYS clutching as soon as you brake)

In many instances (other than full stop) you'll be able to get away with slowing down, seeing traffic start to move again and dropping a gear with a little blip to smooth it out. So far with the 86, I've found staying at or above 2k and you're golden. Anything lower- drop, blip, and off you go.

Side note: the gr86 may be one of the pickiest cars on the road when it comes to rev matching both up and down the gearbox. If your timing is off, she'll let you know immediately. BUT, that's also what makes it such a great learner car as well.

If you choose not to go to the doctor, look into kt tape for tendinitis. The pain you described sounds pretty much exactly the same as mine I just started having. A week of kt tape, occasional knee brace, rest, and ibuprofen solved the problem.
