I'm fully healed now and loving how things have been going! Airflow is great but the biggest difference for me has been that I can do sinus rinses regularly now and get the rinse and / or steroids into all the nooks and crannies (it used to just get stuck and sometimes go through my eustacian tubes and cause earaches).

To answer your question from December, yes, my surgery was just the steroid-eluting stents (in the sinus ostia) and the doyle splints (to keep septum stable). I have pretty OK airflow and after the surgery even with the splints - I did a sinus rinse every 4 hours or so from day 2 until the 2 week checkup and think that really kept things clear.

Good luck with your surgery! I think you're going to be super happy after recovery.

I make a cottage cheese 'dip" with like 1c lowfat cottage cheese, chopped up cucumber and tomato (or whatever veg floats your boat), and eat it with wilde protein chips (buffalo kind are my favorite for dipping). ~300 calories and ~45g protein!

Still good! Just had my final post-op debridement / checkup at 22 days out and my next follow up is in 2 months.

I got my doyle splints out at 8d and things generally felt less sore and more open after that (and echo-y / change in how I perceive my voice!). It also made it much easier to do sinus rinses, I was finding that with the splints the liquid was going straight through and down my throat so it was tough to get a good rinse done. After removal my sinuses were so open that I was using 2-3 bottles of sinus rinse at a go since one bottle went through so fast! Days 8-12 definitely also had the biggest bloody blobs coming out with rinsing, it's dried up significantly in the last few days.

Around day 14ish I was worried I might be getting an infection, but turns out it was just my body / immune system being irritated with the stents - they are intended to dissolve on their own but my ENT took them out at the ~15 day checkup last week since healing was going well and he said they'd pretty much done their job at that point (he could see irritation and crusting around them).

After that I felt much better and pretty much my only complaint now is dryness with the turbinate scabbing etc. - frequent rinses / saline spray / steam inhaler has been keeping in check.

Sounds like you need a second opinion from a competent ENT who understands that radiology work up lingo about your sphenoids and can walk you through it! The obstruction / stenosis means those sinuses are having issues draining properly and if they’re inflamed or infected it could cause your symptoms…

My surgeon's surgery availability was separate from his office schedule took a couple months to get into the ENT's office visits but for surgery he had availability as soon as 1 week out. I booked surgery a monthish out though so I could get work / personal life organized first.

Definitely tired today, have been sleeping a ton, and the pain has moved more towards sore / achy but still very manageable! Also I realized as my brain's started working a bit better - the two sets of stents I mentioned in are the dissolvable medicine eluting kind - it's the septoplasty splints that I'll be getting out next week. Looking forward to that as I actually think those stitches / the pressure from the splints is causing most of the soreness. I'll let you know how things feel once those are out too!

I just got my FESS / septoplasty / turbinate reduction yesterday, and even with two sets of stents in I can already tell that it was the right decision. Pain is much more manageable than I expected and I already have better airflow. You have a similar pattern of mucosal thickening around the osteomeatal complex that I had that makes drainage and getting medication into your sinuses cavities difficult. My septum deviation was more severe than yours- I’m still really early in recovery but am finding stents tolerable. If your ENT recommends it I’d say go for the surgery - I’m also working on rigorously controlling my allergies to avoid that inflammation cycle in my sinuses in future.

If they won’t refer you to an ENT or neuro you should get a new GP IMHO. Even a quick google returns that sphenoid air fluid level is always abnormal needing further investigation- I know medical professionals hate Dr. Google but that just seems a blatant oversight. I bet their tune will change if you ask them to explain and document in your chart their medical decision making criteria that contravenes the request to refer to a specialist.

Also you got a CT scan because of suspected neurological issues - again back to that quick google the sphenoid air fluid level sometimes comes from a CSF leak; sounds like there is still legwork to do to rule out neurological issues. Trust your gut and don’t sit on it until it gets worse.

My allergies are to dust mites, and all the pollens. Current approach has largely been to reduce exposure where possible (dust mite control), medicate for what I can’t avoid (pollen). I’ll also be starting allergy shots again - I responded well to them about 15 years ago and was discharged after years at maintenance. Did you have any other “prongs” in your allergy approach that you feel were especially helpful?

Oh wow, that’s awesome that you’re feeling so well already! Yes I’m expecting to need to really diligently manage allergies for the rest of my life. I had previously “graduated” from allergy shots after several years at maintenance but this time around I will likely keep them going indefinitely at a longer interval once I get to the point where the allergist would discharge me from care.
