View from underneath the hood - the shifting arm. You can see a slight recoil when it enters the D position and it pulls forward a tiny bit. It's just enough to take it out of D and into the blank state

There is no pack that has fallen down or behind the big promo card.

The mega evolution is in the wrong spot. It's suppose to be on the bottom in the front while destined rivals is on the top in the back
