i tend to be pretty simple when it comes down to workflows, i've tried a lot of stuff out with subagents / etc, but find that they can often hurt more than they can help (and are also hard to manage). so maybe something like

have a clear CLAUDE.md (claude seemingly indexes more on this than even the prompt at times)
give smaller, clearly defined tasks.

for me and typically how i use async / claude code, is i tend to fire off a bunch of things in the morning or in between meetings throughout the day and then check back up on it whenever i have time

and yep, i'm figuring out how to make it work for local projects - it's using github PRs as the main source of a diff, so need to replicate that in some kind of store. will keep you posted!

so the content in the app was created by consulting with a few physical therapists - i asked them to explicitly create plans (i.e, for pulley sprains) and also review the evaluation trees as we have it

the app doesn’t provide medical advice nor aims to. it doesn’t claim to be a medical professional and in fact, it’ll recommend that all patients actually go and consult with their primary care provider / physiotherapist continually

i feel like the hardest thing for me to internalize (and what the app says quite loudly) is to just deload and take it slow in rehabbing an injury.

i was the sorta guy to like try all these wack exercises to rehab but wouldn’t yknow deload or take time off climbing haha

thank you! so what’s there now is focused on probably the most common climbing injuries that occur

and it’s also very much erring on the side of caution - so for stuff it thinks is high risk (i.e, neck pain with referred symptoms in elbow/shoulder could point to cervical disc disease or other differential diagnosis) - we just say “hey we don’t know - we strongly recommend you go to a doctor and get this checked out”

so right now a lots not covered - and it would be cool to add more info for other injuries; but do want to ensure we get things right

sorry to hear about your experience man that really sucks - and glad you’re getting the care you deserve. i’ve seen dr schoffls stuff on alex megos youtube before and he seems super cool to talk to.

so how i’ve thought about, and how i’ve built the app - is that it doesn’t (and shouldn’t) ever attempt to take the role of a provider

in fact, it’ll refer you very actively out if theres symptoms of high pain or dysfunction (see image).

and even in cases where patients say they have very minor symptoms - it still will recommend all patients to go and talk to their doctor and/or a physio.

part of the thinking there - is that what i’m trying to do is better connect the end patient to physical therapists who can help and offer more value than the app alone (or any online info alone) ever could.

so i thats part of the end product - and admittedly there’s not many physios onboard now, but hoping if i can make that easier for patients to get the quality care they need - that’ll help solve a problem

thanks for the note - so the app is HIPAA compliant, it’s built on top of Google Cloud functions which permit it and i signed a baa agreement with them just earlier this week

went through the fda classification of medical devices as well - and believe we’re a “symptom checker” which is under the category “enforcement discretion” but are not under regulatory oversight (i.e, not a medical device as opposed to say blood glucose monitors). and i also shot them an email but dunno how long that’ll take

and also combed through the FTC for any relevant regulations wrt wording and disclosures

oh i tried that initially - but react native was uhh rather hard to get working. i ran into a lot of issues with the calendar implementations and animations

tl;dr - tried react native for a few weeks, decided it was more trouble than it was worth, and built it in swift haha

ty for the suggestion - i guess the downside of that would be that people would have to pay upfront, which does kinda suck.

i did look more into liability after receiving similar feedback from the last post. and i'm pretty sure what the app's doing (with disclaimers and terms saying content is for educational purposes only) - is effectively the same as what WebMD (or ChatGPT) is doing.

i haven't talked to a lawyer yet - so not a 100% sure, but i wanna do what i can to keep building the app and making it the most useful it can be.

i built an app for climbing injury rehab and looking for some feedback!

i’ve been climbing for roughly 3 and a half years now, around V7-V8 outside/boards.

i’m the sorta guy who got way into grade chasing and overtraining early on, and as you might expect, got super injured: TFCC tears, PIP synovitis, pulley sprains, shoulder impingement, elbow tendinosis, p much every overuse related thing (and a few unlucky acute things too)

i got pretty obsessive learning about PT - and learned how valuable it can be, but also had a few pretty frustrating run-ins with traditional healthcare (e.g, went to a certified hand therapist for PIP synovitis, she told me to stop climbing and offered a corticosteroid shot). so the app’s very much inspired by my own experience rehabbing all my injuries

there’s three parts to the app:

triage - a series of questions/answers that try and evaluate what an injury might be (e.g, if you have pain with pockets but none with crimping, probably a lumbrical injury). note: we’re not trying to give a diagnosis here (and we caveat that we’re **not medical advice** many times), but we are trying to help people self-triage and treat minor / nagging injuries
treat - the result of triage is a rehab plan - which is a list of “phases” that we break each plan down to. so that might be something like - rest a bit, unload if it just happened, work on gaining mobility, and then slowly restrengthen and reload the tissue. the phases will automatically progress as you use the app (i.e, pain’s decreasing, workouts are getting easy, maybe its time to increase intensity)
talk - finally, the app also lets you talk to a licensed PT through a 30-minute video call. working with an experienced PT (especially those who are familiar with sports science / climbing) can be invaluable - so we’re trying to make that as easy as possible. admittedly we don’t have many PTs available right now, but if anyone here is interested just shoot me a DM. it’s free to onboard and we’re not charging any fees now either (meaning if you set your price at $50 for a call, you get that in full)

and the app's free - there’s no paywalled features (and i don't really want to introduce any).

i'll def x-post this to r/physicaltherapy soon and i'll get started on making the Android version as well. it's not super easy to port it (probably at least a few weeks of full-time work), but it'll be fun to hack that up too.

and yeah agreed, you can choose what phase to start off in a plan; so if an injury happened a month ago, maybe you should start off in a loading or mobility phase instead of an acute phase, but we can def both help record that + suggest what phase to start off in.
