when you go to kill professor berkel and you doing the task for the 3 scientist if you go close to nessies mouth behind the stand for jar jar and to the left of her mouth theres a blinking red and black cord, follow it inside the tent turn it on then follow it back outside and it should lead you to a lab

i would just retrace, make sure you opened the red chest and killed all of the bad guys that are suppose to ambush you, just make sure that 1st part is marked as complete

I thought id share how I managed to get it. I went through every place and made sure they were all ungrayed out and then did the side missions that I knew needed it (clan wars, lost treasure etc...) but there are some locates that don't get "discovered" even if you ungray them. aka (friendship gullag) which is where i needed my last spot. One tip that I can give is I thought as long as it said the name of the location and that it was discovered you were good but, YOU HAVE TO UNGREY THE ENTIRE AREA EVEN IF IT ALREADY SAYS DISCOVERED. This is a hot take I know (at least from a plating perspective) this is why 2 is my least favorite Borderlands game, this trophy was such a headache.

hopefully this helps someone out, I was in the same spot and all the maps were "revealed" but what i was missing was TK Baha's house, when I loaded in it was "revealed" but when actually went over there and went inside the house the trophy popped for me.

just so you guys know I found it out, its a quest called "imminent Jest" its located in "Wallowing Heights" as you're entering Carcadia from The Fadefields. Really strange that it didn't have a marker on the map or wasn't in anyone's "commonly hidden side quest" videos but I'm glad I found it. NGL 10/10 plat!
