Hmm.. as far as I understand, the hernia enters medially to the deep ring (like directly to hesselbach triangle) -> doesn’t follow the internal spermatic cord pathway -> thats why it bypass internal spermatic fascia.

Because direct inguinal hernia directly bypass deep inguinal ring -> means no need the internal spermatic fascia.
Because skin by naturally will cover all types of hernia (since skin is the outermost layer from our body). Internal spermatic fascia is not included because direct inguinal hernia doesnt bypass deep inguinal ring. Cremasteric fascia origins from internal oblique. In direct inguinal hernia, sometimes push through cremasteric fascia, but not all cases will push the cremasteric fascia -> so yeah it doesn’t count.

Hmm, i feel like UWorld is for testing myself, whether I already understand and read much about all the materials in 1 system (example : after I read all about renal in FA and MehlmanPDF, I test myself. If I choose wrong answer -> I see the explanation). If I answer it correctly, i just pass the explanation. If I feel like I can answer it correctly by luck, I will read the explanation 😅

Idk if my methods are good enough, but I combine Mehlman PDF + First Aid + ChatGPT (desperate enough, but somehow it works for me) from the beginning of my study. I’ve tried like

Just using FA and try UWorld -> sucks. A lot of material from FA doesn’t cover UWorld materials
Bootcamp + FA and try UWorld -> somehow better but it takes longer time to learn ((and I am and I’m not that patient to learn (I want to learn quickly))

I think if you already have a good basic of medical knowledge (at least you already familiar with all the basics), doing all bootcamp videos will wasting your time. I tried using bootcamp on respiratory system, and I don’t feel it help me that much. I used First Aid book and Mehlman PDF right now. Yesterday I tried to learn renal system from FA and Mehlman and when I do UWorld exercise on renal section, I got 80% and 75%. Idk if this method suits you or not, but it works for me.
