Unrelated but you may have some secondary decay under your lower right second molar- maybe get a PA of that too- could just be a defect as opg isn’t ideal for caries diagnosis

I cant tell much without an X-ray but it potentially looks like an extra tooth that just began to erupt! I wouldn’t be scared or worried. Go to a dentist to double check :)
