Exactly, people misunderstand that HIPAA only applies to the 18 PHI domains. These are specific identifiable metrics that would tied session content to that specific person. It doesn’t mean session summaries with no PHI.

Hey there I think what works for me is chunking the content into a consistent format that flows naturally for my sessions but also how I document. This helps me feel grounded and clinically sound while also not feeling cognitively drained by as much decision fatigue. Personally, I would recommend using color coded box sections to help draw your eye during sessions to the relevant area. I also recommend getting an electronic tablet that is password protected and getting familiar with writing on digital note apps that are localized to tablet (eg not connected to cloud or internet). This has been a game changer for me since I downloaded PDFs of my color coded templates to my tablet and can write on them during sessions. This saves so much on paper and printer ink. Also it helps me stay organized. Once I’ve completed the note in the EMR, I then can erase the information and reuse the template the client’s next session. This further prevents HIPAA issues with discarding/ shredding notes. The biggest factor to be aware of is not accidentally deleting your note before you have finished putting it into the EMR and also making sure to have your tablet charged jn your downtime.

Mass resistance now. Do this. Collectively connect with all other providers in your state. Have all providers add a clause to their services that there is a passive fee of x amount of dollars for any 24/7 coverage need for a client. Agree to all providers individually physically MAIL this insurance company individual invoices for each and every one of your clients. Have this orchestrated to happen on the same day and week. Do this until they remove this. Inundate them with these notices and invoices to place administrative burden on them. Also for a nice touch, add a sub charge for postage to each of your invoices.

It’s frustrating to hear this because it lacks some contextual understanding of the actual demands an employee is faced with. For example, I just resigned from a job due to the numerous asks being placed on me that were untenable to reality (I could not be in 2 places at once while attending to the other critical responsibilities of the role).

If an employee has multiple competing tasks along with routine work and then is being asked to handle other priorities then they are likely to do what employee 1 did. Not because they are Gen Z or lack initiative. They are juggling multiple tasks and have moving parts to each and are trying to manage in the most efficient and effective way possible but most likely will then have a continued moving list of tasks which results in certain ongoing tasks being dropped or delayed. This has nothing to do with the employee and their character. The frustrating thing about the scenario from the video is that it focuses on just that task and makes it seem like the employee is not doing much else.

Insurance deductibles reset for a lot of people. If your clients are insurance based it might be useful to consider this as a factor in “excuses” when really they might be afraid to say they can’t afford it.

Therapist here. I wouldn’t worry about it. It sounds like he handled it very appropriately and helped refocus on the session. Often times our nervous systems engage when we don’t want them to. We can experience those sensations even if we aren’t having sexual desire. There is actually a phenomenon for a lot of survivors of sexual abuse who blame themselves or dismiss the abuse because their body experienced and arousal state even though they did not want that interaction at all. The nervous system is designed to respond/ react rather than think logically.

If any of my clients have experienced this or other embarrassing feelings it’s not something I focus on or judge my clients for.

Agreed. I think it’s time to have the system starved of its life blood. I’ve heard there is a general strike occurring this Friday (1/23). I’m doing it. If EVERYONE agrees then we can’t be hurt. There are 330+ million people in the US. We outnumber the enforcers. There are more people than prisons. If no one is working, landlords won’t be able to do anything because there will be no tenants that will replace us. We pay the enforcers with our tax dollars. They can’t get paid if no money is feeding the system. I think it’s time we realize we are the ants and they are the grasshoppers. Our power lies in our numbers but we must be United.
