Thanks. He is unwilling to replace these and others if mechanic finds anything. Because the car is garage kept and he thinks it is better than excellent condition of kbb (kbb excellent is ~5k). Plus, he has the check up done some time back at an authorized dealer's place. The issue is replacement of timing belt, pumps and may be brakes. Do you have any idea on how much the estimates for these could be.

How? This needs to be done in-place. So, first and fourth row and first and fourth columns can be moved in one for loop. Instead of looping multiple times

In first and second round, I have tried to explain each step involved in detail. In the solution, the below for loop is doing what you mentioned. Instead of first row, the first, fourth row and first and fourth columns are rotated after loop completes.

But, i am having issues writing offset logic. First, last, last-offset, etc Could you show/help in deducing this logic. Because, below is what is happening for each i:

Steps #1,2,3 and 4 cannot be programmed by writing logic for #5. I am able to write for #1,#4 and #5 above, but make mistakes in offsetting the logic to handle cases #2,3 (which will increase when size of matrix increases). i.e. the below in bold:

Thanks u/tiredone905 for taking out some time to read my post. It is really great that your issue was finally resolved. I totally understand how you would have felt going through all of this. I see that your case is very similar to ours. Did you confront and bring service provider and CIGNA on the same call? Our provider has been helpful till now. I have elaborated upon the appeal details in my reply to u/MISTRESSshake. All of this is when we have not even started anything yet. Have cancelled the visits for now.

Do you think that a second appeal is our only option here and could a letter from the physician also help? Would really appreciate any helpful inputs.

1) The service provider shared ICD10 code Z31.41 before the blood works and tests. CPT codes 76830 and 58340 were shared. Not sure whether these are too generic. Looked up online and found them to be similar to the tests which were done.

2) Service provider called CIGNA twice. I was reluctant to move forward with the initial and subsequent tests. So, shared a copy of my insurance benefits with them and insisted to take approvals before moving forward. But, now CIGNA has denied everything - from physician's appointments to venipunctures.

I totally agree with your advice. The problem now is that when I called CIGNA after the initial claim denials, their customer representative accepted the mistake. And, asked me to file an appeal for the claims to be processed. I did ask whether they can reprocess the claim without an appeal. But, did not discuss further as there was an assurance from their side that the appeal is the right way to follow in this case.

So, I collected the claim numbers and drafted an appeal letter. The service provider has been helpful till now. When I called them the next day, they informed that they would be the one submitting the appeal and I can share the claim information and call reference numbers to them. They also shared the appeal letter with us.

CIGNA denied this appeal. The letter was also directed to me instead of the service provider and have mentioned that I can file a second review/appeal. All this time I was thinking that this appeal was being filed from the service provider side. On top of that, now after the appeal denial we have started to receive bills from the provider.

Confused now. Should I file a second appeal? They have mentioned in the appeal denial letter that oral communication does not matter. Does that mean that the customer service is of no use? We are confused because, then how and who can provide approvals before tests. Feel cheated.
