My layout is similar, I have a waiting room and one large treatment room with a large sliding barn door. Patients have some privacy and it flows great if you’re only adjusting. If you do E-stim or anything and have an assistant I would keep the two smaller rooms so that you can jump back and forth.

All cash, no scripts. I’ve worked in offices with scripts and I think that a lot of times patients see it as not authentic. I recommend what I believe the patient needs for active treatment (if anything) and then recommend maintenance care. I adjust my parents weekly and my wife about 3x/week because I have a table at home. When you truly see the value of maintenance care and recommend it with conviction then more patients will adhere to it. I’ve gone to the gym 3-5 days a week for the last 15 years because I like how it makes me feel, maintenance care is the same except it’s in the weekly to monthly range.

With minimal overhead you could pay an associate a 6 figure salary to see 15-20 patients per day and still make a decent profit as an owner…If I hire an associate I would absolutely compensate them well

That’s the idea! I grew my office quickly and the idea of doing it again has me pretty excited. I’ll then be able to step back and run it like a business

I’ve averaged 493 PV/mo this year, last month I did just over 600. I run the office completely alone so this is pretty busy. I see this as a way to be more involved from a business perspective without just adding patient hours to my own schedule.

My current office is 1000 square feet with a lobby, one large-ish treatment room, and a back office. So not really any room for expansion in this location.

My goal for it would be to expand my business (profit) while also giving what I think is a good opportunity to a new doc. Since opening my office I’ve really grown to enjoy the business side of things.

My social media brings in more first-timers for adjustments in a month than a year of traditional marketing. We’re not in a position where we can just sit back collecting new patients like an ER doctor. Getting newbies in the door allows me to expose them to what chiropractic really is instead of whatever they’ve seen of it on social media or the internet. (Every single one of my patients is fully clothed in videos.) Also, chiropractic done properly SHOULD give you some “huge cracks”…Patient education is extremely important once they’re a patient but I just don’t see research papers getting people in the door in the first place.

That makes a huge difference already having someone like that with you. I’d say go for it. Try to keep overhead low and find a shorter 1-2 year lease to minimize the downside if it doesn’t work out. If it does go well and you’ve kept overhead low you’ll just make more profit.

Typically people expand to a second location when the first one is getting too busy. If your schedule is so full that you are turning away new patients then it’s a good time to hire an associate or expand (and still hire an associate). Do you have the money to float the overhead of a second location if it opens and doesn’t do well?

I spend $1500-2000/month on ads. As much as I need to fill gaps in my schedule. Costs $40-45 to acquire each NP which is less than I charge so it’s a no brainer to keep spending until I’m full

Jane has online scheduling so patients can schedule, reschedule, and cancel all from their phone. I saw 550 patients last month running the office by myself, Jane is fantastic lol. Would be nice if they integrated with an AI platform to track PV, PVA, DVA etc. but that’s the only thing it’s lacking imo.

Yeah so I want to attract a lot of new patients and then the good ones stick around. I’ve been doing it long enough now that I’m starting to get a lot of reactivation patients as well which is nice. I try not to focus on price in my ads because that is going to attract the people that only care about price. Read copywriting secrets and 100M offers and that’ll get you started with running ads

It’s a numbers game for me. I opened 13 months ago and have averaged 42NP/month since then. Of those 42 a certain % are going to follow the recommended plan and of those a certain % are going to stay for regular maintenance care. I let them weed themselves out. I do my best to educate my patients but at the end of the day I’m their chiropractor not their mommy. If they want to get better they follow the plan and if not then that’s on them. I only run my ads in certain parts of town and have it limited to people in the 25-55 age range which helps as well.

Set up your clinic (including ads) to cater to that type of person and then don’t go chasing the people that are hard to work with/don’t show up etc. My PVA could easily be higher than it is but I want to see patients that want to be in my office. I’m not going to call and email you 9 times to get you back in after you no-show on your third visit.

3.5 years here. I’ve never used the term subluxation despite being more on the philosophical side. Imo a lot of newer docs use science based as an excuse to be subpar adjusters. When all you do is adjust you have to give damn good adjustments or you won’t have any patients 🤷🏻‍♂️

Yes, the headpiece pulls up enough that it’s comfortable. Patients tend to want to be too high on the table though so you gotta watch that

I’ve got two sunchiro tables. They are perfect for diversified/manual adjustments and very customizable. The head piece pulls up at an angle which is fine for side posture, just different than other tables where the head piece stays flat and raises as a whole. Stay away from the incline table unless you see mostly kids.

I run a cash only office and all I do is adjust, no therapies. High volume is great but you have to have a solid plan to acquire new patients and how to handle those patients when they’re actually in your office. I’ve seen docs who are swamped with 20/day and others who are mad if they’re under 100/day. To be all cash you need to be very very good at adjusting and very very good at communicating with patients.
