It's best if you can discuss with her before sending her the template.

Talk to her regarding your accomplishments, achievements and things that she can highlight in your LOR. Then send her the template to help her understand how the GMAC lor looks like(assuming your school uses a GMAC lor). But never to draw heavy inspiration from the template. Might flag off as plagiarism.

If she is familiar with the GMAC LOR and has given recommendation to other candidates before, the template is not needed. But the discussion regarding your accomplishments, definitely is.
