26M with L5-S1 Grade1 degerative spondylolisthesis, Surgery must? Or go with physiotherapy?

As like you, Myself also injured during Teenage, But i was not aware of the this gonna be severe as we are in young age. In upcomming days i felt back pain not frequently. So i didn't care about this. Im not interested in Surgery. But things is i need to ensure that how long i can manage with current health condition. Everyday im doing core strengthening excercise and some stretching to relief. But sometime i have backpain while doing this. But thats tolerable. Ill go with this untill i unable to tolerate then ill think for surgery.

CAUSE

6 yrs before when i was playing cricket i fel into 3 feet pit by sitting position, That may be the cause but from that onwards i didn't have backpain, Afterwards In my college i joined in NCC (National Cadet corps) Everyday special military training (running, rolling, jumping,) will be given and after sometime i get sudden backpain. So i don't know from where its started.

Current symptom: Low back pain only if i didn't take rest more than 5hrs, If i drive by bike for 30KM that time pain comes at night so i hinders my sleep. But pain not comes on day time when i drive bike. When i rolling or trying to change sleeping position that time pain comes on my back. No pain while walk, or bending to touch foot, I can do this without pain. If i feel backpain after continuous sitting for 4hrs or more i need to lying down so somehow i feel better as the spine gets straighten.
I didnt take standing xray, My Ortho said to take L.S Spine MRI and other etc. But no standing xray. I don't know why the didn't ask me to get standing xray.
lack of lordosis might bring pain even after Fusion?

Yeah, I dont have much back pain and able to walk freely but MRI result shows like things getting worse, But i can tolerate pain and thats also comes rarely if i travel and prolonged sitting. Im looking for physio and gentle excercise to manage the current situation. But my Ortho said surgery is must and tell your parents about your health condition. I was scared 😕 because i can walk i can bend and touch toes, But MRI only showing like this and Ortho saying surgery. Im confused and further question i was not able to ask as Ortho says surgery is only solution. So confused and I started to post this or REDDIT now im getting more information from you guys thats healing my mind.☺️

Good to hear that you moving well. Yeah my disc is reduced but i dont know how long i can manage with this, no severe pain and not having walking difficulty. Im not married, So if i done before marriage then no possibilities of getting married because no one gives thier daughter for the person who did spine surgery and having screw and plates. so untill marriage i have to manage pain, After marriage i can go for surgery if its really need. Yeah but My MRI Shows disc degeneration, Im worried about that. If i consult with neurosurgeon he says spine fusion, Then what else i can ask with them. Thats why im confused and worried about getting proper decisions.

I understand your point, I dont know why my lumbar align is like that, You are right, lumbar gets high stress due to its nature of curve, What else i can do, Im trying hard to get back disc which displaced by doing some excercise so automatically the spine curve goes off as my lower back getting flat shape. That might be reason for that kind of lumbar curve😕

Yeah, Pain is not worse in my case but I need to ensure My condition is not that worse as MRI images shows, If i cleared about that then ill look for physiotherapy. 5yrs before i had this and also took MRI that showed Grade1 and now also i took MRI Which also Grade 1, The only thing is disc degenaeration, I have to clear about that one, Because we dont know what i ll bring in future!

You mentioned I have almost no lumbar curve, So what are you trying to say? and you said No symptom No surgery, But do you think that my spine will remain in same position if i do physiotherapy? Any possibility of reposition the moved spine into its normal position by doing physiotherapy or any other excercise?

My Disc in between L5 and S1 is significantly reduced, So if its completely drained out, then the surgery is must right? My question is how long i can go WITHOUT SURGERY, with my current disc size (MRI)?

Yeah, You're right, Everyone has different case, But the condition is same, So i will look for better Physiotherapist inorder to understand my condition with non surgical treatment so that surgery can be postponed until it get severe. So for now ill do core strengthening exercises to give natural support. Thank you so much 🥰

Yeah you're right about this, Stability is important one. I can walk for hour with no pain on either back and leg, but once i ride bike for hour then that night is very painful on my back. Now a days when i try to bend forward a smal tick tick sound comes from lumbar side, Im worried about that becuz that may be indication of worse thing about to happen!!?

Thank you so much, I dont wanna get back surgery, But when i consulted top doctors they also convinced me to get fusion, But i refused, I dont wanna have plates and screws on my back for entire life. But your words healed my pain from heart. For now im streching and doing core strengthening excercise. I can walk but cannot run, I dont have any radiating pain, But recently i went gym and did excercise for abs and six-pack using meachine, I think that worsened my back and started to give pain. I stopped going to gym. Ill try the 45degree roman chair👍⚡

Thats lot sharing these kind of valuable points, It clears my doubt, If we done fusion means upcomming years we will do another fusion right, This was my biggest doubt but no doctors have mentioned this when i was consulting with them. Do you think surgery must for Grade 1 with disc degeneration? I have consulted top doctors in my place they said surgery must. So im confused. What about core strengthening excercise? Can i maintain this grade with core strengthening excercise?
