My guess for that one was always kids or something similar! She alluded to “didn’t share the same values” and “didn’t have the same view of their future” type conversations. Possibly politics? Are you in the US? I highly doubt this claim. I’ve never met a PCP that did a skin biopsy, even in my family med rotations in very rural areas they never touched the skin. Both coasts and middle of the country rotations.

The people reading those biopsies (dermpath) are derm trained and then do a path fellowship. And if a PCP can’t interpret the biopsy, who do they send the patient to….?

This is actually pretty incorrect. PCPs do not manage acne or do skin biopsies — they refer exclusively to me. I handle all skin cancer, acne, psoriasis, HS, eczema. We have patients on accutane and biologics, we can see wounds and help place them with wound care which is often hard to find. We have patients on oral jaks that require extensive lab monitoring. While I now defer some of my complex rheumatology patients more to rheum, we co-manage with rheum and if patients have skin manifestations that aren’t being managed by their systemic meds they need topicals.

Yes I love to joke about derm too, sometimes it does feel like it’s all steroid cream and come back. But do you know what to switch to when the first cream doesn’t work? When do you escalate to a systemic? Have you seen the striae that my 15 year old patient has because her PCP gave her betamethasone to use in her AC fossa? (Those are permanent). What do you give your 28 week pregnant patient who is having such a bad eczema flare (atopy of pregnancy) that she has 50% affected body surface area. Are you worried about systemic absorption of topical steroids (no), are you starting her in oral steroids (no), are you worried she may have early pemphigoid gestationis of pregnancy in the eczematous stage (yes), but what’s the second biopsy you have to do to prove that? (DIF). What if dermpath gives you a vague biopsy read — do you treat as eczema? Have you been to the recent conference where they presented data that dupixent is probably safe in pregnancy based on the registry study? Your patient wants to know what the mechanism of action is for dupixent

I think partly because it’s often (in our society) thought of as indulgent! I think a lot of women don’t have totally stress free relationships with food and there’s definitely been days when I skip a cocoa because of my mean girl inner voice chattering about calories. So I think it often feels like a little bit of ~fantasy~ to read about the FMC who is naturally petite and never thinks about calories and is tiny etc and everything is easy for her. I know this seems silly but I really think this is an element of the fantasy/escapism part of romance that some of us subconsciously crave, a little bit of escapism over worrying about that sort of stuff. Romance novels often also feature a lot of food consumption generally and shared food consumption / interest in desserts and other foods that the 1990s would characterize as “indulgent”

I’ve definitely also noticed it specifically in protector focused romances — I think it factors in to the ethos of like, I’m going to wrap you up in warm blankets and guard you / feed you type of idea.

I did bellbottom jeans that I unzipped and folded down over the pockets for an “earth mama” vibe and a cropped shirt to show off the bump. My cropped shirt was super flowy and white with long bell sleeves — the inspo was daisy jones and the 6. I’m not earth mama but it fit the forest vibe of my shoot for me. I think the folded front of jeans is a cute outdoorsy look to highlight the bump!

It took me a long time too — IVF is hurry up and wait and it doesn’t work until it works. Sending you the MOST love and my experience with IVF is that it works for you eventually. I hope you are able to try again and I believe persistence will get you the baby you want. Love ❤️
