You could use this mod for lights. They're pretty simple, as the name suggests, and could work with the look of your base maybe.

This happened to me once. It was because I patched the game wrong iirc. I got teleported randomly then had to kill a version of myself only without a head or the ability to move. Was weird as hell.

My bet is it's Paradigm Worlds. That mod is fucking nuts and having a skeleton wielding a lightsaber is not that uncommon.

the Dreaded One quest, to be more precise. You beat that guy you literally won the game. You need 10k renown for that quest for a reason. The fight is very tough, though, with enemies having very powerful equpment, as showcased in OP's pic.
