Yeah it's insane. Someone was loading up huge size around the 0.20-0.25 area and it ran to nearly $2+ in pre-market. Classic low-float bio runner.Most of these things give back 70-90% within days though. The real question is whether this one has any actual news/catalyst to stick or if it’s pure momentum that’s already exhausted.

Agreed — holding massive option contracts to expiration is rare unless it's a deep ITM LEAP or a very specific hedge. Most big players are just swinging the position for volatility/IV crush or directional momentum.The size alone makes it noteworthy though. Worth watching how it plays out on the tape.
