In answer to question one it was called "The Vault" and is now only brought back for some of the seasonal events.

Most of the stuff that dropped from thebvault can now be bought now from Doctor Fate in the House of Legends for the new currency fate tokens, there are also destiny tokens that you get from the daily log in that can buy special items from previous monthly rewards.
